All Firelands Physician Group Primary Care Offices Now Nationally Certified in Patient-Centered Medical Home (PCMH)


February 17, 2017- Sandusky, Ohio -- All eleven of Firelands Physician Group’s primary care practices have achieved Level 3 Patient-Centered Medical Home status with the National Committee for Quality Assurance (NCQA). The offices include:

The Patient-Centered Medical Home (PCMH) is a team approach to healthcare led by the primary care physician. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care. It provides a healthcare setting designed to improve coordination of care between patients, their primary care physicians and, when appropriate, the patients’ families.

The role of Firelands Physician Group in PCMH allows patients to:

• Have a comfortable, confidential environment to talk about healthcare concerns

• Coordinate their healthcare with all members of their healthcare team

• Receive test results and have their questions answered in a timely fashion

• Be kept up-to-date on necessary vaccines and preventative screenings

• Discuss, create and decide on a treatment plan that is best for the patient’s health needs

• Communicate clearly so patients can understand any health issues and related treatment options

“NCQA Patient-Centered Medical Home recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that Firelands Physician Group has the tools, systems, and resources to provide its patients with the right care, at the right time.”

To earn recognition, Firelands Physician Group demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. Not only are all Firelands Physician Group primary care practices now PCMH certified – they all achieved certification at Level 3 – the highest level possible.
